How to get a Delivery Instruction Slip (DIS) booklet from Zerodha?

To receive the DIS booklet, you'll need to follow these steps:

Submitting the DIS Request Form:

• You must submit a DIS request form in the pdf format. Here’s a copy of the DIS request form for Zerodha.

• Once your request is received, the DIS booklet will be dispatched and sent to you via courier within 48 hours.

Instructions for Completing the DIS Request Form:-

1. Client ID: In this field, please enter your BO ID.

2. Collection Instructions:

• Collected by Hand: If you plan to visit Zerodha's head office, choose this option. Remember to submit a self-attested ID proof along with the DIS request form.

• Additional Booklet: Select if you need an extra booklet due to a lost Annexure B. Otherwise, send Annexure B to the head office.

• Canceling Unused Slips: Choose between not canceling or canceling unused DIS slips. Specify the slip number and reason for opting for cancellation.

  
  

• By Authorized Representative: If someone else collects the DIS booklet, ensure the representative submits the form and ID proof signed by all account holders.

3. Correspondence Address: If applying for a new DIS booklet, choose this option for delivery to your registered communication address with Zerodha.

4. Name and Signatures: Remember to include the names and signatures of all account holders.

The DIS request form offers both online and offline submission options:

• Print the DIS request form, fill it out, and sign it.

• Scan and e-sign the form.

• Create a ticket, attaching the e-signed form. Submit within 30 days from eSign.

2. Offline Submission:

• The offline method is required if the account holder's Aadhaar isn't linked to a mobile number or the client is a non-individual account holder.

For First-Time DIS Booklet Requests:

• Please enter the DP ID on the DIS request form (PDF) if you request the DIS booklet for the first time.

• If you need an extra DIS booklet, fill out Annexure B, which can be found on the last page of your existing DIS booklet, and courier it.
